Children’s Community Complex Care Service
Who are we?
The Children and Young People’s Complex Care Team is part of the Manchester Children’s Community Team.
We provide care for children and young people who have underlying medical conditions and require care teams and nursing support in their own homes.
The service is adaptable and care is provided for children and young people with a range of conditions including ventilation and those with acquired brain injury.
What do we do?
The team works with each child/young person and family to provide individualised care packages to a high standard.
The aim of the team is to maintain normal family relationships within the home and for the child/young person to remain safely cared for within their own home setting, and to minimise the need for hospital admissions by prompt intervention of the care team.
The Children and Young People’s complex care team is an adaptable service that works with each child and family to provide a high standard of individualised care. We coordinate complex care packages in liaison with the referring hospital, identifying the clinical needs of children/young people and their families.
With the correct support the child will be able to participate in family and community activities, lead ordinary lives and enjoy their childhood.
Complex conditions which the Team treats include:
• Cancer
• Leukaemia
• Complex Cardiac conditions
• Organ failure
• Non-specific degenerative conditions
• Spinal muscular atrophy Types 1 and 2
• Cystic Fibrosis
• Branton’s Disorder
• Osteogenesis type 11
• Rare metabolic disorders
• Rare syndromes.
